Request For Replacement Certificate
Here you are able to request a replacement or lost certificate.
Please note there is a £15 admin fee to cover the cost of reissue.
Please ensure the following information has been included on the submitted form.

Type of course.

Tutors name.

GCD number if applicable.
On submitting your information a £15 fee will need to be made before the new certificate can be issued.
